The roots of OpenNotes trace back to 2010 when it initiated its inaugural study with patients at Harborview Medical Center in Seattle, Beth Israel Deaconess Medical Center in Boston, and Geisinger Health System in Pennsylvania. To everyone's delight, patients overwhelmingly embraced this practice, and the concept took off like wildfire.

Prevent Medical Errors

Medical errors are one of the top three causes of death in the United States, with adverse drug events accounting for approximately 700,000 emergency department visits and 100,000 hospitalizations annually. Access Your Medical Records and Perhaps Even Contribute

The path to accessing your medical records is usually via an online patient portal where you may communicate with your providers, view your medical records, and manage appointments and bills. This is also where you'll find your visit notes.

If you're a part of the UW Medicine system, accessing your notes is a straightforward process. All you need to do is log into your eCare patient portal, click on the "visits" tab, and select the notes option.

Soon enough, you may even be empowered to contribute to your notes during your appointments.

OpenNotes initiated a pilot study in 2017 to explore the feasibility of allowing patients to type their concerns into their electronic medical records before an appointment. Research participants typed their agenda into the electronic medical record before their consultation. The notes were then stored and could be viewed alongside the provider's notes in the patient's electronic medical record.

At study's end, 79% of patients and 74% of clinicians agreed the process improved communication during consultations. Moreover, 73% of patients and 82% of clinicians expressed interest in continuing the practice. Elmore strongly supports the co-creation of clinic notes for better patient care.

"Patients enter information I wouldn't have expected," remarks Elmore. "I appreciate their involvement, and it helps me provide better care."
